Advert Band 8a Specialist Clinical Psychologist OR Band 7/8a Preceptor post This is an exciting opportunity to join the Lancashire and South Cumbria Specialist Support Team (SST); a developing and expanding service, based in Whalley, Lancashire, and part of the Specialist Learning Disability Division at Mersey Care NHS Foundation Trust. We provide a specialist health service to individuals aged 16 plus and adults who have a Learning Disability and/or Autism and who present with a risk of offending or who have a history of criminal conviction, as well as individuals who are described as having complex challenging behaviour. The service is based on the principles that underpin the Transforming Care initiative; that services for people who have Learning Disabilities should be configured around and with local communities, with strengthened support closer to home and reduction on the reliance of hospital care. The SST focuses on supporting people to live in their own communities, providing robust, personalised care and support to people to prevent admission to secure accommodation both locally and out of area. The team also works to enhance the discharge process, ensuring that people who do need in-patient care stay for only for as long as they need it. The SST provide expert care and wrap around support to individuals and their support teams upon discharge from hospital and work with community providers to ensure the correct training and support is available. In addition, the team provide Early Intervention Support to prevent crisis and support people to stay at home with the right care and support that they need, at the right time. Working directly with individuals and acting as a resource to locally provided community learning disability and adult mental health teams, the SST model best practice in personalised care and support planning in addition to supporting local teams to plan creative and flexible supports. The service is underpinned by the principles of positive behavioural support, ensuring that person centred practices provide the foundations of all support. The successful candidate will demonstrate excellent clinical, leadership and communication skills and be motivated to developing new ways of working. They will have a clear commitment to person-centred practices and be able to demonstrate this in their work with individuals, families and teams.
