Job Description

We are looking for a dynamic and fresh thinking individual who is passionate about delivering outstanding healthcare within the medicine therapy team at University Hospital Southampton NHS Foundation Trust (UHS). You will be experienced in complex discharge planning, with knowledge of a range of conditions and acute rehabilitation skills. The candidate would have relevant experience of team leadership and service development, who values teaching and supporting others within the team. ABOUT THE TEAM This post is within the Medicine Therapy Team, who work on a range of our medical wards which include Respiratory, Hepatology, Gastroenterology, Renal, Infectious Diseases, General Medicine, and patients recovering from ITU. The caseload includes supporting patients with a diverse range of acute presentations alongside long term needs including PD, MS, ETOH excess, COPD, CKD, learning disabilities, and self-neglect. WHAT YOU'LL DO As part of the Medicine Therapy Team, you would be responsible for providing assessments and treatments to the acutely unwell person and evidenced based rehabilitation of longer term conditions. This post will offer: * The opportunity to work with a diverse caseload including bariatric patients, complex social histories, complex pressure relief and posture management, complex cognitive and behavioural presentations and supporting acute rehabilitation needs for those with long term conditions. * Completion of diverse assessments and treatments including complex discharge planning, specialist equipment provision, multifactorial cognition and functional assessments, and management of mood and engagement. * Experience of building strong inter-professional working relationships across all care providers both internal and external to the hospital. * Opportunities to engage in up and coming service development projects, be innovative and influential to enhance the services we offer. You will be supported and encouraged by the band 7 team with a keen interest in developing the medicine therapy service. * Teaching and learning development of others including supervision, supporting preceptorship, apprentices and students. * Endless opportunities to explore career development and CPD activities where you will be encouraged and supported to develop extended skills and advanced practice in acute medicine. * Engage in in-service training and peer support including within the medicine team and wider Occupational Therapy team * Support of a friendly, diverse and forward thinking interdisciplinary team. WHAT WE'RE LOOKING FOR This post will include 7 day working and therefore the post holder will be expected to participate in weekend working on a rota basis. You will be a car driver, with full driving licence, or ability to travel between hospital sites or for necessary patient visits.
