Job Description

Service/Ward/ Department Summary The aim of Bury OPCMHT is to promote positive mental health care for older people whether they are living in their own homes or in a care setting through a comprehensive, holistic assessment of needs. Most service users will have complex and enduring needs however the team also provides short term crisis management support. The team also carry out Nurse led memory clinics within the outpatient department. Role Summary To manage a caseload of secondary care service users and also provide short term crisis management as part of the Duty role. To provide comprehensive assessment of service users and support/advice to carers. To plan and implement care plans and risk assessment/management plans for individuals. To support the nurse led memory clinics as required. Key Responsibilities of the Post To conduct Specialist mental health assessment and Trust approved risk assessment/management of service users and devise appropriate care plans /risk management plans. To provide evidence based therapeutic interventions appropriate to the service users assessed needs and promote choice and service user involvement throughout. To support the nurse led memory clinics under direction of the team manager and to participate in the Duty System ensuring all referrals are dealt with in a timely manner. To be responsible for a caseload of clients with secondary care complex needs. Key requirements of the Person Specification RMN or equivalent health care qualification To meet the requirements of the NMC in respect of fitness to practice and CPD Experience of working with Older people in a mental health setting and experience of working in a community mental health team. Evidence of MDT working and post qualification experience of supporting clients with mental health issues. Knowledge of organic/functional conditions.
