Community Psychiatric Nurse - Older Peoples Mental Health Team

Job Description
Within a multi-disciplinary team you will have responsibility for assessing, planning, implementing and evaluating the care needs of older adults with Mental Health problems. Emphasis will be on care and treatment using validated psychological approaches and working as an integrated service to meet complex needs of this client group. You will be responsible for providing supervision to untrained staff and nursing students. Experience in Older adults Mental Health is desirable. You will provide a service to a defined geographical area for patients who are experiencing mental health problems and also has a key role in promoting and developing the role of the Community Psychiatric Nurse. You will provide high quality, culturally sensitive and effective specialist nursing to older people with mental illness living in the community by working in partnership with service users, care providers and other agencies. You will provide triage, assessment, ongoing interventions and evaluation of these, participates in discharge planning processes and facilitates the coordination of care working collaboratively with patients using a biopsychosocial model within community Older People Mental Health services. You will be required to identify patients with complex needs requiring referral to psychiatric inpatient services, other agencies/teams both primary & secondary and voluntary agencies. It is essential you are a Registered Mental Nurse (RMN) with live NMC registration and have evidence of effective leadership skills commensurate with the Band 5 role.
