You will be working with adults aged 18-64 who live in the Ashfield/Mansfield locality who have complex physical needs and long-term conditions. This can also include co-morbidities of learning disability, mental health diagnoses, or substance misuse. You will have the responsibility for the assessment and provision of a wide range of services, including: equipment, moving and handling, rehousing and major adaptation. You will have an exciting opportunity to joint work with our Social Work colleagues to ensure the promotion of independence and reduction of dependence upon care provision through early assessment and provision of OT solutions. You will also work in collaboration with our wider team, our local housing providers, equipment providers, our health colleagues and any other relevant agencies. You will be fully supported to maintain your CPD portfolio and will have opportunities for ongoing professional and personal development. You will need good I.T. skills, and experience of working independently in the community and managing your own caseload. Knowledge of specialist equipment, DFG processes and complex moving & handling is desirable. Regular professional supervision will be provided. Video interviews will be held on the 29th June.
