Pharmacy professionals have an opportunity to provide first class care and services to patients in varied roles across our integrated system. The role We are looking to recruit pharmacists into this new and exciting way of working to deliver care to the residents of Dorset. There are two post types including Primary Care Network roles, and working in integrated services working with community and primary care services. Our current recruitment needs centre around Purbeck & Bournemouth locations & Weymouth. Opportunities for part time working will be available. In primary care roles, you will provide clinical leadership in: * Medicines optimisation working closely with practice management to deliver key prescribing objectives in structured medication review * Care home clinical services * Antimicrobial stewardship * Personalised pharmaceutical care, transfer of care * Integrated care with community services * Prescribing safety and governance. Our services across all stakeholders employ a number of pharmacist prescribers and you will have opportunities to share learning and develop links with specialist pharmacists across the county of Dorset, with automatic membership of the Prescribing Academy. You will use your prescribing skills to manage patients with complex polypharmacy and chronic disease. Initially you may be working from a practice environment or from a central support network where your skills will be used in the delivery of care from various settings. Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of 40 and the Trust will recover this cost in instalments from your first 3 salary payments.
