Role: Case Manager – Specialist Services
As a Case Manager, you will be responsible for ensuring the appropriate treatment and high standard of care for our clients within their homes. Your role will involve working with commissioners on new packages, identifying suitable staff for each package, and providing continuous support to both staff and clients. You will also be responsible for meeting clinical and social expectations of our clients by building positive relationships, implementing care standards, and reducing hospital admissions through case management techniques. Compliance with CQC regulations and the implementation of good practice will be essential to this role.
The Case Manager's specific duties and responsibilities include analysing clients' physical and mental health needs, collaborating with clients and stakeholders to create personalised care plans and conducting risk assessments for clinical interventions when needed. You will be serving as a communication point between various parties involved in the care package and reviewing care packages to adapt to changing needs of the clients. You will be providing support to clients and their families, conducting staff supervisions and appraisals, participating in internal training programs, staying updated with clinical knowledge through professional development, and attending additional training as required by the business.
Essential requirements:
· RNLD/RMN Qualified (minimum 2 years)
· Proof of eligibility to work in the UK
