Clinical Nurse Specialist - Community Respiratory Team
Overview
Job Description
This post is fixed term for 6 months. You will provide support and timely intervention to patients with chronic respiratory disease in the community. You will prevent unnecessary or unscheduled acute hospital admissions and provide specialist respiratory care to patients with COPD. You will also provide clinical expertise and support to healthcare colleagues working in respiratory care field. You will support the early discharge of COPD patients from hospital and link with the Respiratory Early Supported Discharge (ESD) team, and hospital based respiratory assessment services/respiratory nurse specialists. It is essential that you have a Degree or Diploma in nursing and should be currently registered with the NMC. You should have relevant post graduate experience including respiratory experience. This is a 6 month fixed term post, working full time hours of 37.5 hours. The shift pattern for this post is Monday - Friday 8.30am - 4.30pm.
