Job Description

The Pioneer Group are working together with North Birmingham Primary Care Network to recruit a new Social Prescribing Link Worker to help people find and use community support, services and activities. This will be through face-to-face appointments in GP surgeries, home visits and telephone calls. We need confident self-starters, able to work in busy environments while building good relationships with a variety of health and social care professionals. This will be a rewarding role giving you the opportunity to make a real difference. Not only will you directly help the people we support to improve their lives, indirectly you will be doing vital work to ease the pressure on the National Health Service and free up resources for everyone in the area. The successful candidate will be providing wellbeing support, advice and guidance to vulnerable adults, therefore we would like to hear from candidates who can demonstrate experience of offering this type of support to people with mental health/learning disabilities. You will be a confident communicator who is comfortable working with a wide range of people at all levels. This is a busy, varied role that will require you to work closely with both vulnerable people as well as GP's and practice managers. Therefore, you will need to be proactive and organised with the ability to manage a busy workload.
