Are you a highly motivated and experienced ACP? Would you like to live and work in beautiful Dorset? We are looking to recruit 1 highly skilled band 7 ACP into a new and exciting integrated approach to deliver care to the residents of Dorset. In collaboration with Castleman Health care LTD, East Dorset and North Poole Primary Care Networks, we are offering an opportunity for an ACP to be involved in a truly integrated way of working to deliver high quality health care. As part of the Networks you will be joining a multi-disciplinary service to provide rapid response and admission avoidance to our patients. You will be working with the team in service delivery, triaging and prioritising visits to achieve our overall objectives. The ACP will be based in the East Dorset Hub at St Leonards but working flexibly across East Dorset and Poole North Poole. You will provide advanced clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis and advanced decision making skills. Ideally, the successful candidate will be an independent non-medical prescriber or willing to complete this training. Hours will be full time at 37.5 per week but may be scheduled over an 8am-8pm with a possible future weekend rota as the networks develop.
