About Us We are recruiting for a Part Time Job Share (21 hours Wednesday-Friday) Occupational Therapist who is motivated, forward thinking and dynamic to work within the North HEART Team. HEART is an innovative Housing Adaptation Service formed as a partnership between local district councils in Warwickshire. The Role The OT will undertake assessments with a range of customers who have a variety of needs. The OT is to be solution focussed and have a knowledge of housing adaptations, related legislation, assistive technology and equipment to enable customers to maximise their independence in all aspects of daily living in their home environment. The role with involve: Carrying out functional, risk and standardised assessments. Developing support plans with customers and carers Undertaking a variety of tasks to deliver grants in accordance with policy. Working with clinical and non clinical colleagues to deliver housing solutions. For further information please see the Job Description and Personal Specification Key Requirements Qualifications - Holds DIPCOT, BSc or equivalent in Occupational Therapy and be HCPC registered with no impending sanctions. Experience in delivering housing solutions to a range of customers and the Disabled Facilities Grant. Able to travel throughout Warwickshire as required to meet service demands. Competent in using computerised customer recording systems and other IT based systems.
