Job Description

The Collaborative Care Team is a domiciliary rehabilitation team mainly serving the local population of Castlepoint and Rochford. The team provides an alternative to hospital admission and supports discharge from hospital by enabling people to regain their independence in their own homes. We are in the position of having several part time (2 x 20 hour) Rehabilitation Assistant vacancies. The successful candidates will be expected to work a range of shift patterns over seven days per week, in line with service needs. This is a friendly, supportive team and we are looking for highly motivated individuals to join us. We welcome applications from experienced staff from a health or social care background, who can contribute to our high standards of care. You will have the opportunity to gain specialist knowledge in rehabilitation and the skills if necessary, but in return you must have excellent communication skills, are enthusiastic, be flexible and willing to learn.
