Job Description

Our Complex Intervention Service (CIS) is a short term rehabilitation and reablement service offered to people in their own homes. The purpose of the service is to provide a mix of care and rehabilitation to support individuals regain their independence following sudden deterioration. We are pleased to offer several part time vacancies for new Intervention Facilitators (otherwise known as Rehabilitation Assistants) to join the team and continue to allow us to provide an excellent service to the people of Southend. Our goal is to facilitate a safe and timely discharge from hospital or the prevention of admission to hospital, supporting people using tailored programs to aid recovery where ever possible and continue with their lives. Our service is available from 7am to 11pm, 365 days a year including Bank Holidays and all new staff will be required to work a range of shift patterns including weekends. Our usual contracts are for 30hrs per week, completed over 5 days in 6hr shifts, although applicants interested in a contact for fewer hours could still be considered although they would still be required to complete 6hr shifts. Current vacancies are 30 hours per week - 14,121 - 15,216 per annum pro-rata If interested you should ideally have community experience in rehab/reablement, however extensive training will be provided to successful applicants including those with little or no experience. This is to ensure you have the necessary skills to provide a high quality service. Applicants must be flexible in relation to duty rota's and available to attend mandatory/competency training or meeting, some of which may fall outside of normal shift patterns.
