Rehabilitation Nurse - North West Rehabilitation Service

Job Description
Working within the North West Rehabilitation Service and occasionally including the intermediate care unit, this post offers an excellent opportunity to work within a multidisciplinary team providing assessment, treatment, intervention and advice to older and disabled adults with multiple pathology aiming for hospital admission avoidance and supporting hospital discharge The post holder will work autonomously in a diversity of health and social care settings taking on, in addition to the professional clinical role, case co-ordination, health promotion, audit projects, and the provision of expert advice on professional interventions. You will manage a complex clinical caseload within the service, applying specialist knowledge and skills relating to their professional role and working as an effective and efficient partner in the interdisciplinary team. This is a community post and you will work in a variety of locations in the NW. It is essential that you are currently a Registered Nurse with the NMC and that you have a current driving licence. Having experience of being part of a multi-disciplinary team and working in the community is desirable. The shift pattern for this post will be day shifts.
