Job Description

A new Senior Rehab Support Worker post has become available within the Community Rehab Team in East Dunbartonshire HSCP. The team provides assessment and rehab to adults over 16 years old within the East Dunbartonshire area where person centred, goal focussed input is provided within individual's own home. You will carry out baseline assessments, using an agreed framework and present the findings to the multidisciplinary Team to enable decision making with regard to future Team interventions. You will implement treatment programmes for patients, make assessment of progress and provide advice to patients and their carers on a range of patient conditions, forms of therapy providing empathy and reassurance. You may also be delegated appropriate tasks by professionally qualified team members including Physiotherapist, Dietician, Occupational Therapist, Speech and Language Therapist, Nurse, and Team Leader. Training, supervision and support will be provided to enable you to work within their agreed competence levels. You should have attained or be working towards a SVQ in Health and Social Care or equivalent and have good communication skills. A current Driving Licence is required for this post.
