Job Description
OCCUPATIONAL THERAPIST (BAND 6) KINGSTON CRISIS AND HOME TREATMENT TEAM About the team: The team is a multidisciplinary, community based mental health team that aims to provide a safe and effective home based assessment and treatment service as an alternative to in-patient admission, and for early discharge from the ward in the least disruptive and restrictive environment. It works in conjunction with, and is integrated with, other local service elements. The workload is team-based and team members do not carry a specific caseload. The Team is available to provide care and intervention 24 hours per day, 365 days of the year for people who are experiencing a mental health crisis of a nature or degree that would require admission in the absence of the CHTT. Home Treatment Integrated Out of Hour's service Through the sharing of borough based staffing the service has the capacity to provide a timely response to crisis out of hours. The response will include assessments at A/E, Sec 136 suite, home visits according to the night. Staff will undertake rapid assessments, refer and signpost to other services. The Out of Hours activity will be coordinated through the Acute Coordination Centre. About the role: The post holder for Kingston will be based at Tolworth Hospital. There is an expectation that the post holder is able to work shifts, including night duties. Key areas of responsibilities:- * engaging clients in crisis * participating in rapid assessments at home ,and in A & E departments * co-ordinating effective gate keeping assessments when needed * providing a range of bio/psycho/social interventions for service users and carers * planning for robust and sustainable discharge and aftercare * involving clients in planning and evaluating care given * working as part of the team to achieve the best possible care for clients About the location: KCHTT is based at Tolworth Hospital, Red Lion Road, Tolworth, KT6 7QU. Nearest train station is Tolworth Railway Station and Surbiton Railway Station. There is a canteen/restaurant on site where food and beverages are available; it has both hot and cold food. There are cafes, restaurants and shops in the local area, providing variety of menu. Parking is available on site (subsidised fares for all substantive staff), also in the local area.
