About Us Warwickshire County Council has an exciting opportunity to recruit 2 full time Occupational Therapists to join the Reablement team. The post will be based in the South (Heathcote House, Leamington) The posts will be for 37 hours per week. There will be an expectation that the post holders will be required to provide countywide support to both the North and South Reablement teams. As a Reablement Occupational Therapist your role will be to carryout comprehensive assessments with customers who are struggling to manage activities of daily living in their homes or who have returned home from hospital following a period of ill health. You will setup a support plan with the customer to enable them to relearn the skills needed to function more independently, reviewing and amending as required. You will identify ongoing areas of need and refer onto the appropriate service(s) as required. You will work as part of a team that consists of Occupational Therapists, Occupational Therapy Assistants, Reablement Officers, Reablement Supervisors and Reablement Assistants. You will also provide professional support and guidance to the team and you will receive professional support from a Senior Occupational Therapist. A You will work within the requirements of the care Act 2014, HCPC codes of practice and other legislation and guidance. Reablement focuses on promoting independence by supporting customers to do as much as possible for themselves both physically and psychologically. Reablement aims to do this by supporting customers to continue living in their own home, preventing admissions into hospital or residential settings, and facilitating timely discharges. We believe that by supporting customers to identify and achieve their personal goals and outcomes, and by embedding personalisation, customers will have choice and control over how their support will be delivered, this will have a positive impact on health and well-being. Reablement is a short-term service working with customers for up to 6 weeks. The Role You will be working within the Service to respond preventatively to referrals for social care and support and will use Occupational Therapy interventions and a strengths-based approach to enable adults and older adults to remain in their own homes with the optimum level of independence and to enable / support customers to achieve their desired outcomes. You will be responsible for managing your time and work load and will be expected to work closely with your colleagues within the team and external agencies /services, as well as independently. You will also act as a point of expertise to the wider multidisciplinary team, providing support on complex cases and carrying out joint assessments. All posts are permanent. For further information please see Job Description and Person Specification Key Requirements The post requires you to work a shift pattern of 9 am - 5 pm and 12 pm - 8 pm Mondays to Fridays, one Saturday in every 7 weeks with the possibility of this increasing to Sunday's depending on demand of the service. There will also be requirement to work bank holidays on a rota basis. A minimum of twelve months post qualifying experience required.
