Yes Well you are the person we need You will be a core member of the Nursing & Care Support team, providing a comprehensive service to both patients and their families and working closely with other members of the wider interdisciplinary group in delivering individualised therapy programmes and interventions. You will be required to regularly review and update protocols, risk assessments and care plans and monitor the progress of individuals on your caseload against specified goals. You will get the opportunity to work alongside other healthcare professionals such as Nurses, Physiotherapists, Occupational Therapists, Assistant Psychologists and other medical practitioners. Your main responsibilities will be: Working as part of an interdisciplinary team alongside clients and their families, carry out established clinical therapy programmes to meet the needs of clients experiencing functional impairment. This will include demonstrating and teaching activities as guided by the IDT. To attend, and where applicable contribute to, handover meetings between shifts. Carry out personal care as needed, such as bathing, grooming, dressing, oral hygiene, and toileting needs whilst ensuring adequate nutritional intake. Escort and support patients to other departments, hospitals and on external visits/trips. So about you …. ideally, we want ? Good communication skills and the ability to work in collaboration with others effectively. Responsible, reliable and flexible approach to work. Level 2 in Health and Social Care is desirable but not essential.
