We are currently recruiting for Adult's Social Workers to work within a Hospital Discharge team for Leeds . Duties in this role include: - Completion of Hospital Discharge Assessments - Utilising DST and CHC checklist - Liaising with a variety of health professionals To apply for this role you must be a Social Work England-registered Social Worker with a student placement or post-qualifying experience in Adult's Services.
