Overview

Reed Social Care are recruiting for Hospital Discharge Social Workers in Leeds. Paying up to £24.78 Hour (PAYE) and £30.98 (Umbrella) The role consists of facilitating safe discharges and liaising with members of community services, both statutory and voluntary, to arrange services for patients. Assisting patients and families and helping resolve any social, practical or emotional difficulties that impact on their ability to cope with their illness. Hospital Social Workers will be required to provide support, information and make referrals to a number of relevant services if needed and being able to offer Information on benefits and entitlements, housing and social welfare. Requirements: The candidate needs to be an individual who is social work qualified and registered with the HCPC.
