Overview
Reed are currently recruiting for an Assistant Social Worker in a fast paced Hospital discharge team in the Leeds area for an immediate start. Discharge planning is a vital part of case management in social services, and your role will be to ensure the patient receives the right care even after being discharged from the hospital. Candidates will be expected to have a social work qualification with knowledge of relevant legislation, policies and procedures.
