Overview

Qualified Social Worker Reed Social Care are currently recruiting for an experienced social worker to work in the Discharge Team based in Leeds . This team is based in a multi-disciplinary team with health professionals and other disciplines to work together providing an holistic assessment and level of support for discharge. Duties include Assessments in accordance with the Care Act 2014 and Mental Capacity Act 2005 To facilitate discharge from the acute hospital setting. Knowledge of safeguarding, delayed transfers of care is beneficial. The team also perform a duty function and offer support and assessment in the Emergency Department to prevent reduce and delay the need for formal support and services. £250 referral bonus - if you aren't interested, maybe you know someone who is?
