Overview
We have a brand-new opportunity for a Occupational Therapist to join our fantastic team working alongside Southampton Hospital. This role is part time (22.5 - 30hrs) healthcare at home a little about us Our vision is simple, we help our patients have their best day, every day. Working alongside healthcare providers and pharmaceutical companies, we ensure first class clinical care in the home to our patients and provide greater insights and efficiencies to our customers. Delivering through a patient centric approach, coupled with the introduction of user friendly technology, we provide clear visibility of the patient's adherence and persistence, resulting in improved outcomes. All this contributes to supporting our customers to identify interventions which enables patients to rediscover 'their normal' and have their best day every day. What does a Occupational Therapist do at healthcare at home? Conduct accurate holistic assessments of patients' needs in accordance with approved model of care. Assess, discuss and agree upon functional goals related to daily living that are important to the patient such as washing, dressing, domestic chores, bathing, driving, preparing and eating meals, or participating in work, school or leisure activities. Educate patients and give advice on the safe use of functional equipment, ensuring correct equipment is ordered, delivery and fitted correctly. Complete medical observations as per agreed clinical pathways within scope of practice, for patients in their home. What skills, qualifications and qualities do I need to be successful? Fundamentally we are looking for clinicians who are dedicated to offering outstanding professional patient care, it's at the heart of what you do. You will be a registered occupation therapist who will work as part of a multi-disciplinary team responsible for delivering care excellence in line with patient's needs, key performance indicators and outcome measures. Deliver effective and high quality occupational therapy to patients in a home environment, or in the community, to help maximise their function and level of independent living. As this is a regional role travelling to visit patients in their home setting, therefore it is crucial that you have full UK driving licence.
