Mersey Care Medicines Management Team Liverpool Community Division is a well-established team of pharmacists, technicians and support staff with a track record in delivering outcomes. The service underpins medicines safety and evidence based practice within the Trust by supporting community services e.g. safeguarding in care homes, policy and PGD development, non-medical prescribing, and governance role for bed based services. However the majority of services provided are those under contract with Liverpool Clinical Commissioning Group as follows; * GP prescribing support to 88 GP practices across Liverpool * Falls medication reviews * Interface and medicines commissioning support * Care Homes We are looking for an enthusiastic pharmacist to join our progressive team. You will influence the quality of prescribing and medicines use for patients through becoming an integral member of the GP practice team, assigned to a group of GP practices. Completion or part completion of a postgraduate diploma/MSc in clinical pharmacy is expected or equivalent clinical experience. An independent prescribing qualification is desirable. Evidence of continuing professional development is needed. You will need to work independently and part of a team, conduct medication reviews and demonstrate excellent communication and influencing skills. The role is supported by senior pharmacists, technicians and support staff. There are excellent opportunities for development within the role and the wider team.
