Job Description
You will be joining an expanding Locality Pharmacy Support Team working to support safe, appropriate and cost effective prescribing in the 9 General Practices in Dumfries and Nithsdale. The function of the Pharmacy support team is to promote safe, effective and economic use of medicines and to facilitate integrated patient-focused care that meets the present and anticipated needs of the population of NHS Dumfries & Galloway, in accordance with local and national strategies. The focus of this post is to work as part of this highly successful team, based in general practice, to support the delivery of pharmacotherapy, as defined by primary care transformation in Scotland. You would focus on high quality and cost effective prescribing, which is patient-centred and tasks would be directed and supervised by the locality lead pharmacists. Active participation in, and contribution to, multi-disciplinary and multi-professional teams in a fully integrated manner would be expected. You could also potentially work with patients in their own homes, care homes and other healthcare settings to provide clinical care which will support and improve access to primary care health services. You would embrace all the objectives set out by 'Achieving excellence in pharmaceutical care' and there would be the opportunity to develop clinical skills, for instance independent prescribing and undertake polypharmacy medication reviews where appropriate, to support the work of the senior team in optimising pharmaceutical care for the Dumfries and Upper Nithsdale community.
