*** CURRENT EMPLOYEES ON NUH ONLY*** We are looking for a pharmacist to lead our service to the team providing palliative care at NUH. You will work with the hospital palliative care team across both campuses and directly with the Consultant team to provide excellent medicines care to our patients. You will provide direct patient-facing care to our patients with pharmaceutical needs at the end of life and support the ongoing development of systems and processes to manage medicines across the Trust and the wider healthcare system as we move towards integrated care in Nottingham and Nottinghamshire. You will develop as an independent practitioner and take on an active role prescribing and deprescribing within the palliative care team including a commitment to seeing patients in the day therapy unit and supporting the Consultant-led MDT ward round at Hayward House, an operational pharmacy commitment within the Cancer Services team and time to support the development of guidelines and services across the service. You will provide expert level, complex, pharmaceutical care for patients, optimising their medicines and ensuring the effective transfer of care. You will be supported to advance your specialist knowledge and effectively utilise your non-medical prescribing skills. Key responsibilities of the role include: * Acting as the lead pharmacist for the palliative care team * Providing a clinical pharmacy service within the cancer team * Involvement in education and training of a variety of healthcare professionals including diploma & pre-registration pharmacists, and medical and nursing professionals * Lead on development of the service and wider work across the healthcare system * Support weekend working with the Pharmacy department * Participate in back-up senior on-call rota supporting junior pharmacists What we look for: * The successful candidate must be highly motivated, a team player and a capable clinical pharmacist with good interpersonal and leadership skills, with aspirations to develop as an independent prescriber * As a specialist pharmacist you will have excellent communication, organisational and leadership skills * Experience in project work and service development. Willing to help develop the service through development of clinical guidelines, and aiding with financial and risk assessments to support new services * The ideal candidate will be a clinical pharmacist with a prescribing qualification and a broad experience of working in palliative care and cancer What we offer: * Supportive and friendly teams, both within Pharmacy and in the wider admissions MDT.
