Job Description
Occupational Therapist - Dealing with Adults Community Cases NonStop's client is a local authority based near Brent that are seeking an experienced Occupational Therapist to joining their hospital service. The successful candidate will work closely with the multidisciplinary team to ensure that the service promotes independence in line with the Local Authorities values and behaviours. The role will require that the candidate has a relevant qualification in Occupational Therapy as well as being HCPC registered. Requirements for the role: - Provide Case/Care management and close working with the enablement care provider. - Undertake effective functional assessments of the individual/carers needs and risks to encourage independence, provide support and prevent harm. - Make Recommendations and set goals to enable function. - Address identified needs to promote well-being in accordance with the Care Act 2014. - Provide/recommend equipment and adaptations to support safety and independence. - Complete moving and handling assessments and plans including assessment for single handed care. - Provide advice and support to multidisciplinary colleagues.
