Team Lead Occupational Therapist or Physiotherapist for Frailty 43,712 - 49,325 per annum incl. HCAS West Middlesex University Hospital Site We are seeking a motivated and energetic individual to drive forward the Frailty service at West Middlesex University Hospital. The successful candidate will be based within the Medical Rehabilitation and Frailty team, but will interface across the entire Frailty pathway to optimise outcomes within this cohort. You will be joining at an exciting time with the frailty service currently undergoing a large multi-disciplinary re-design, including the establishment of the Acute Frailty Unit. There will be ample opportunity to be involved in the development and proof of this large and highly visible piece of work, and you will be supported to take a key role in the implementation of best practice as we drive to meet local and national policies. You should have good general experience gained within an acute NHS trust, and will have developed and mentored more junior staff and students. You should not be afraid of challenging the norm, and should embrace change as a positive presence that ensures we are consistently providing the best possible care to those who need it. You will be self-motivated, and be able to work flexibly across multiple environments including A&E, AMU and the Older Adults wards. You will work closely with key stakeholders from multiple disciplines including consultants, specialist nurses and divisional service leads.
