Job Description

West Hertfordshire community respiratory service provided by CLCH is looking for an enthusiastic forward thinking respiratory nurse specialist to join our friendly expanding multi-disciplinary team. The post holder will provide patient centered integrated care, providing specialist services including hospital at home (admission avoidance scheme), home oxygen assessment & review service and home visits to provide expert specialist care to patients with COPD, asthma, bronchiectasis, OSA and ILD. Working effectively within a multi-disciplinary team and demonstrating effective communication skills is essential.
