An exciting opportunity has arisen to be part of a vibrant team as a Practice Lead within Change, Grow, Live Waltham Forest. You will be working with people to help them change the direction of their life, grow as an individual, and live life to its full potential. You'll work with people who use our services, helping them get to where they want to be. The Practice leads work with complex service users who struggle with dual diagnosis, health and social care issues. This role is about partnership working, leading on developing service level agreements, include an outreach approach to building partnerships and engaging those with complex needs. The role includes managing a smaller case load of complex service users, training professionals and taking a leadership approach with the team alongside team leaders Responsibilities We are looking for a passionate and committed individual to work with team leaders to lead our team. Responsibilities include leading team meetings, delivering training. Taking a lead role whilst managers are absent, line managing social work students. Being the single point of contact for mental health, housing and social care teams. As well as ensuring staff follow Change Grow Live policies and they may carry a small caseload. The successful candidate will be expected to work flexibly within an agreed number of hours to maintain the appropriate level of service delivery and you be required to work over a number of bases within Waltham Forest as per service need.
