Community Support Worker - CERT/District Nursing - Stratford Locality Stratford Locality - 37.5 Hours We are looking for a flexible and organised individual to join our team. You will have at least 1 year related experience, preferably in the community and have some knowledge and skills in managing the care of long term conditions and end of life patients. The successful candidate will be self -motivated, enthusiastic, possess excellent communication skills and be a good team player. You must be able to demonstrate effective interpersonal skills. You will be working collaboratively with the day service teams and the Community Emergency Response Team to deliver seamless care across the locality, and in particular for our end of life patients. The post is a community post and the ability to meet the travel requirements of the role is essential. The community nursing service operates across the hours of 8.30 am to 22:00pm, 7 days a week, including weekends and bank holidays. You will need to be able to work flexibly and be comfortable with agile working.
