Job Description
The Palliative Care Team provides multi-professional palliative care services for patients cared by Southend Hospital. The Team has highly developed collaborative relationships across primary, secondary and voluntary care. As the Macmillan Palliative Care Clinical Nurse Specialist (CNS), the post-holder will play a critical role in supporting patients and their families who have complex needs. This includes working alongside ward and clinical teams in all adult settings ensuring that individualised care for patients is consistently of a high standard that both coordinates and delivers equitable holistic care. We are looking for a highly skilled professional to join the team. Where possible the successful applicant has experience as Palliative care CNS and is also non-medical prescriber, however training is available for successful applicants that need to develop these skills. A passionate commitment to supporting palliative and end of life care in hospital settings is essential. Practice innovation, self-motivation and excellent communication skills are vital to this challenging role. This role requires a dynamic, confident nurse leader who, as part of a multi-disciplinary team, can influence practice by demonstrating excellent clinical leadership and motivational skills.
