Staff Nurse Band 5 - Community inpatient Services At Guy's and St Thomas' we champion care for our staff, as well as our patients, ensuring they are supported throughout their professional journey. Minnie Kidd House Located in Clapham, Lambeth, MKH is a 28 bedded specialist unit offering specialised care within a homely environment. Our patients have a variety of complex needs such as tracheostomy care, neurology, diabetic management, and end of life care. This complexity also includes intense and unpredictable medical and nursing needs. Considering this, MKH provides continuous care over a 24 hour period, 7 days a week. The unit is also supported by two Consultant Physicians, a Pharmacist for medication management, and other key specialist-services. Pulross Centre The Pulross Intermediate Care Centre is located in central Brixton. Located within an award winning building, the centre has a total of 20 beds and a strong multidisciplinary nurse-led team. 14 of these beds are dedicated for 14 intermediate rehabilitation where admitted patients have clear, identifiable rehabilitation goals. At Pulross, the following are strived for daily: * Reduce hospital admissions * Provide rehabilitation post-acute admission * Prevent early admission into long-term care In September 2016, the unit opened the final 6 beds for level 2b neuro-rehabilitation as part of the KHP South East London Neuro Rehabilitation Service (SELNRS). The Amputee Rehabilitation Unit ARU is a 12 bedded unit based at the Lambeth Care Centre, Kennington. Admitted patients undertake a six week intensive amputee rehabilitation stay. All patients (aged 18+) will have at least one major amputation, and varying levels of functional ability. This can be rooted in a range of reasons, including: vascular disease and diabetes, trauma, sepsis, and oncology. ARU includes an indoor gym, a fantastic landscaped garden, and community access visits. Promoting independence, our patients are challenged daily to meet their goals, designed and assisted by a multi-disciplinary team, providing an enabling care approach. Nurse led, a medical team supports three days a week, working closely with the clinical lead. In addition, a Personal Discharge Facilitator provides an effective safe discharge plan for the patients. With strong link to Guy's and St. Thomas' acute services, the unit provides high standards of specialist care. This includes: complex wound management, including vac therapy and debridement, diabetes care, falls management, and counselling and psychological support. The caseload is challenging but extremely rewarding with no two days the same. At ARU, you will receive bespoke in-house rehabilitation training from the clinical and nursing lead, physiotherapist, occupational therapists, and tissue viability nurse. With a highly dedicated team of only twenty nursing staff, ARU covers Lambeth and the South East. We have vacancies across the sites detailed. Please follow the vacancy guidelines on how to apply.
