Integrated Discharge Service at UCLH Band 5 Nurse Opportunities UCLH has a number of new and exciting vacancies in the Division of Integration. The Integrated Discharge team is led by the UCLH Integration Operations Manager with management and support from a Matron and Band 7 specialist Sisters covering the services of Simple and Complex Discharge, Homeless support, Admissions Avoidance in the Emergency department (7 days) and Alcohol and Drugs specialist service. The team work on the wards every day providing an expert discharge support service to ensure safe and timely discharge. They also work with in the Emergency Department team each day assessing patients, and by working with the ward teams and our community colleagues support the patients discharge and prevent readmissions. This service may also include outreach visits to patient's homes. The team work a shift system over seven days a week and are seeking Band 5 Qualified nurses to expand the team. In order to be successful in the role, all applicants must have a keen interest in improving the patient experience on the ward, and care about the continuity of the service between the acute and community setting. You will have good knowledge and experience in the acute setting and knowledge of community services is desirable. The ward support for discharge will involve leading and contributing to the co-ordination of patients' discharge arrangements and assist in the administration processes relevant to discharging patients from UCLH. The team work with the ward multi-disciplinary teams and Social and Health care providers to prevent potential delays to discharge by identifying patients complex functional, social and home environment needs prior to transfer. The Band 5 nurses work as part of the team, under the supervision of the Matron in charge of the service with supervision for the band 7 leads and band 6 senior staff nurses. The nurses will support the Integrated Discharge Service Matron in the safe, efficient and effective running of the Patient Lounge and the overall Discharge Service. S/he will rotate within discharge services to cover the Patient Lounge (Discharge area) and ward based discharge support and will supervise the Nursing Assistants and receptionist when based in the Patient lounge.
