Overview
A top performing West London local authority are looking to bring in an Approved Mental Health Professional/ Mental Health Social Worker ASAP for a 4 month fixed term contract. The purpose of this position will be to undertake work with clients with severe and enduring mental illness. This will include carrying out assessments, including risk assessments of clients. You will work within the trust/ local authorities Equal Opportunities Policy. Principal Duties of the Post: To make assessments of need and risk assessments, following the Care Programme Approach, with clients and carers in order to formulate care plans, including crisis care plans. To undertake the Care Co-ordinator role for an allocated caseload of clients with severe and enduring mental illness. To work with their families or carers as appropriate. To carry out care management duties in accordance with the Care Act and have knowledge of and be able to apply other relevant legislation including Children Act 1989 and Mental Capacity Act 2005. To undertake statutory duties under the Mental Health Act 1983 as required. To attend AMHP forum meetings. In order to be considered for selection you will need A Dip. SW, CQSW or equivalent qualification recognised by General Social Care Council and sufficient experience of working with people with severe and enduring mental illness sound knowledge of the Mental Health Act 1983 and other relevant legislation.
