We offer a £250 referral bonus per successful candidate that is referred Temporary pay rate: £34ph ltd Hours and length of contract: •Full time Monday - Friday 09:00 -17:30pm •The manager is looking for the position to be filled ASAP with an initial 3-month contract length (likelihood of extension) Daily Duties: You will work as a specialist mental health clinical pharmacist to support the medicines-related care of patients suffering from mental health difficulties as an active member of specific wards to support the safe and effective use of medicines. Daily duties will include: •Ensuring safe and appropriate use of medicines in order to maximise benefits and minimise risk to patients - regular review of prescriptions, identification of drug interactions and adverse effects, advice on clinical monitoring, compliance with legislation •Provide a clinical pharmacy service to assigned wards, outlying mental health units and/or to teams caring for outpatients oReview prescriptions for accuracy and legality and liaise with prescribers about actual or potential problems oLiaise with the dispensary when required, to facilitate safe, secure and effective dispensing and medicine supply oFacilitate the effective outcome of treatment with medicines oMonitor patients for potential and actual adverse effects of their medicines oEnsure that patients understand the purpose of the medicines oSupport the planning of patient discharge with respect to medication •Make appropriate referrals to other members of the multidisciplinary team •Advise medical and nursing staff on the actions, properties and the safe, cost effective use of drugs •Ensure adherence to the requirements of the relevant clozapine patient monitoring service. •To assist with the development and provision of clinical pharmacy services to CMHTs and other community mental health services. Person specification: The applicant must be DBS cleared and have previous experience in the field.
