Overview

PURPOSE We are ambitious about what the individuals in our care can achieve and therefore focus on the Care Pathway, positive goals and the person’s journey onto the least restrictive services. The Senior OT is responsible for the delivery of effective Occupational Therapy services across multiple sites for adults residing in community rehabilitation units who present with learning disabilities and complex needs. To contribute to the high standards of person centred care provided within this company, utilising Occupational Therapy skills to effectively assess, treat and provide ongoing evaluation of clients. The Occupational Therapist (Band 6) will work as part of a multi-disciplinary team as an autonomous professional according to the College of Occupational Therapy ‘Code of Ethics and Professional Conduct’ and the ‘Health Professions Council Regulations’. 1. To receive delegated referrals from Lead OT, independently manage and prioritise that case load. 2. To work autonomously and independently with access to sessional, clinical supervision. 3. To devise, implement and evaluate treatment programmes. 4. To complete comprehensive assessments of clients, using clinical reasoning to select appropriate assessment techniques which lead you to identify the client’s strengths and needs.This will involve seeking and sharing personal and potentially sensitive information with the client. 5. To agree appropriate and realistic goals for treatment with the client and so produce a comprehensive treatment plan in order to achieve these goals. 6. To delegate appropriate case load to Occupational Therapy Assistant/s (OTA), helping prioritise and ensure programmes of therapeutic activities are implemented. 7. To have the ability to work without direct supervision in the community rehabilitation units delivering client programmes adhering to local and national professional standards whilst keeping the Lead OT informed. 8. To assist with developing and delivering relevant OT training as part of the internal training programme.To ensure personal training is completed on an annual basis. To be a proactive member of the multi-disciplinary team and other agencies (e.g. Social services, Voluntary agencies) ensuring that effective communication and liaison takes place at all times. 10. To be a qualified OT, HCPC registration and membership of the British Association of Occupational Therapists (BAOT). 11. Relevant experience working as an OT, with LD or in a secure setting. MAIN DUTIES AND RESPONSIBILITIES General 12. To provide specialised client-centred occupational therapy intervention to clients (aged 16 or over) who are resident in the community and are referred due to complex needs. This includes clients who may present with complex physical, cognitive, psychosocial and emotional needs due to pre-existing complexLearning disability and Autism backgrounds. 13. To work as an autonomous practitioner in managing a defined caseload of complex cases, in a community setting. To evidence problem solving and clinical reasoning skills in line with evidence-based and client-centred principles. To formulate a diagnosis and treatment plan designed to reduce the impact of disability, from an assessment of the client’s needs. 14. To be an influential member of the multidisciplinary team, providing expert, integrated, client-centred, goal-orientated rehabilitation. 15. To provide supervision, teaching and appraisal for junior occupational therapists, support workers, students and other members of the multidisciplinary team 16. To work collaboratively with members of the multidisciplinary team and contribute to the planning, development and evaluation of Sequence care group’s service provision. To continually engage with and contribute to the development of the provision of occupational therapy within community services. SPECIFIC RESPONSIBILITIES Clinical 17. To be professionally and legally responsible for all aspects of own professional activities. 18. To adhere to and apply the Code of Ethics and Professional Conduct for Occupational Therapists (College of Occupational Therapists 2010). 19. To independently manage a caseload of complex cases providing specialist occupational therapy assessment and intervention to clients with complex physical, cognitive and psychosocial needs arising from their medical conditions. To provide this service in the most appropriate setting to the client e.g. clients’ homes, workplace and other community settings, working closely with the client, his/her family/carer and other professionals in doing this. 20. To be responsible for prioritising occupational therapy referrals within Sequence Care Group to ensure that clients with urgent needs, are seen within appropriate timescales. 21. To establish a therapeutic relationship with the client and his/her family/carers to gain their participation in the therapeutic process. 22. To assess for and diagnose any functional, physical, cognitive, perceptual and sensory deficits using specialised standardised and non-standardised assessment batteries. 23. Based on the analysis of the assessment and a risk analysis, to determine occupational therapy aims and treatment goals, in collaboration with the client/family/carers. 24. To develop highly specialised occupational therapy treatment programmes in collaboration with clients, carers and other involved professionals (e.g. Psychology, speech and language therapists, district nurses, physiotherapists, home care agencies). 25. To plan, coordinate and deliver client-centred interventions on either an individual or group basis, using the principles of graded activity and a range of treatment techniques e.g. functional and remedial activities, splinting, equipment provision, education and health promotion. 26. To assess the need for special equipment/adaptations in line with the eligibility criteria. To be responsible for provision of equipment, conducting a risk assessment and any necessary training (e.g. how to use a hoist), to ensure that the client and their carer’s are safe in its use. 27. To provide specialist advice on lifestyles, role, functional abilities, self-care and the physical environment after taking account of the effects of the medical condition on the individual and his/her family/carers. 28. To provide emotional support to the client and his/her family/carers in coming to terms with the impact of a new or ongoing diagnosis and its implications for the client and family on their daily lives. 29. To use clinical reasoning, critical thinking and reflection to monitor, evaluate and modify treatment, evaluate outcomes and thus ensure effectiveness of intervention. 30. To prescribe and coordinate intervention to be carried out by others, such as rehabilitation support workers or home carers, and to train them to carry out therapeutic programmes as appropriate to the home or community environment. 31. To participate in client related meetings as required e.g. CPA meetings. 32. To communicate effectively and work collaboratively with medical, nursing, support staff and therapy colleagues as well as outside agencies to ensure the delivery of a coordinated multidisciplinary service. 33. To provide specialist advice to staff within the service and to other health care professionals outside the multidisciplinary team, in relation to managing and treating clients within the area of specialist knowledge. 34. To provide verbal and written information to other multi-disciplinary professionals regarding occupational therapy interventions. 35. To maintain accurate and up-to-date documentation this is consistent with legal, professional and organisational requirements. 36. To maintain professional links with occupational therapy colleagues, Occupational therapy assistance and clinical lead occupational therapist regarding occupational therapy clinical governance and professional issues. 37.
