Overview

Position: Mental Health Practitioner Location: Ealing Duration: 3 Months We're looking for a Qualified Social worker to improve the outcomes for forensic psychiatric patients and their families/carers and the public, while working in partnership with other agencies to ensure smooth care pathways and expedite timely discharges. Main duties and responsibilities: To monitor and assure high professional standards of practice and performance in all aspects of service delivery. To contribute a forensic social work assessment of patients being considered for admission where an appropriate social care issue has been identified. The assessment and supervision of forensic psychiatric adult and adolescent patients in order that statutory duties towards such vulnerable adults/children are met. To undertake follow up social supervision of discharged patients in the community as required, To recognise the risk indicators of different forms of abuse and neglect and take a lead on safeguarding within multi-disciplinary team on issues relating to safeguarding adults and children in accordance with the relevant local policy and procedures. Essential: Professional social work qualification Registered with Social Work England Post qualification experience of working in a statutory forensic mental health, personality disorder or learning disability setting.
