Overview
Are you a qualified Social Worker with experience working in Hospital Discharge? Do you have experience of carrying out assessments and reviews? We have a new exciting opportunity to join a tight-knit team based in Powys, paying between £30-32.00 per hour. Benefits : A dedicated consultant to guide you through your recruitment experience Competitive pay rates Flexible hours A friendly and supportive working environment Responsibilities that you will have during this role : Conducting assessments and reviews of the cases given within the team Applications to the panel for funding ongoing care where it's needed Carrying out reviews and care plans for service users Helping them transition into moving out of a hospital setting and into residential care.
