Southwark Council are seeking to recruit a Qualified Social Worker to join a well-managed team in Hospital Discharge. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels. The responsibilities of the Social Worker includes: * Holding a caseload of complex casework the team * Rota as part of the multidisciplinary Internal Discharge Hub which triages all referrals in the hospital and oversees the discharges of non-complex cases via a discharge to assess pathway * Supporting discharges from the hospital and also covering out of borough hospitals in circumstances where a Southwark resident is currently an inpatient * The main function of the team is to support the discharge of those service users whose care needs have changed due to their admission into hospital. This will involve assessing for new packages of care, increasing existing generic care package, supporting with transition from hospital into residential or nursing home care and completing screening assessments for those with rehab and reablement needs * Complete Mental Capacity Assessments and Best Interest planning in addition to overseeing and leading on safeguarding enquiries in situations where the concern has been raised during admission * Safeguarding Adults enquiries * Strong ability to adapt to different discharge pathways and processes To ideal candidate will have: * Social Work England registration * Eligibility to work in the UK * Social Work qualified (MA/BSc/DPSW/CQSW/MSc/BA) Working with Ackerman Pierce you will be guaranteed weekly payment and access to continued profession development, including a dedicated consultant to support you through the interview and on boarding process.
