Clinical Transfer Coordinator
Overview
We have a new opportunity for a Clincial Transfer Coordinator in Southampton . Ultimately, we are looking for someone who will be an on-site expert, taking charge for ensuring the smooth and timely discharge of patients. As a Clinical Transfer Coordinator, you will take responsibility for assessing the suitability of patients and guiding them through the journey from hospital, back home. Meet capacity and contractual targets in a timely manner, whilst always maintaining high degrees of safety and delivering the highest standard of care. Complete clinical assessments, determining patient’s suitability, compiling care pathways and then transitioning care to the wider healthcare at home multi-disciplinary team. Establish and maintain exceptional relationships, internally and externally with both stakeholders and patients to improve performance and assist the growth of the business. Accurately documenting all patient care and clinical information to provide real time data and clinical history to ensure quality care is maintained and delivered. Maintain and demonstrate up to date knowledge, ensuring any gaps in clinical competency are addressed whilst also staying compliant with all business policies and procedures. What skills, qualifications and qualities do I need to be successful? Fundamentally we are looking for someone with strong interpersonal skills who can establish rapport quickly. We also require you to have a commercial awareness and to be able to both influence and motivate people. You will be a registered nurse that has previous experience undertaking holistic clinical assessments. Due to the nature of the role you should be highly organised and be able make decisions and problem solve in a fast-paced environment. This is an autonomous position and so you will need to be able to work confidently and decisively without supervision.
