Overview

Seven Social Care are looking for a temporary Assessor to work with Lambeth Councils Placement Support Team. This position is initially for a period of 3 months and is paying between £17.00 - £21.00 per hour via an umbrella company. The role: To undertake assessments, support planning, identification of resources and reviews in order to assist in maintaining the independence of adult social care service users. To complete appropriate assessment forms, create and maintain records, reports and letters. To identify if there is a need requiring referral to Adult Social Care or Health staff for a specialist assessment and refer as necessary. To coordinate and review ongoing packages of support and to provide care coordination role for service users as appropriate. Key Responsibilities: Undertaking reviews of service users in nursing and residential care homes in and out of borough. Undertaking safeguarding enquiries or provider concerns in order to improve the quality of care in care home placements in Lambeth and block contract homes. Completing Court of Protection work where disputes arise following a review regarding placements. Supporting changes to placements e.g. people moving from a residential to an extra care placement, nursing placements or making the arrangements for a Continuing Health Care assessment. Applicant Requirements: Qualifications: N/A. Experience: 2 years working within a similar role. Compliance: Current DBS check. Travel: Clean UK driving license with the ability to commute to and from work independently. Hours: 35 hours p/w.
