The Role As Waking Nights Support Worker, you will provide short-term, community-based support to patients and Carers living in West Sussex as part of the Emergency Response Service. The service prevents unnecessary admissions and facilitates early discharge from hospital, with a clear focus on positive patient experience and maximising the availability of hospital beds. Your responsibilities will include: Taking referrals for the Emergency Response Service and completing assessments, support plans and providing the required care Providing on-call duties outside of office operating hours if required during your shift Ensuring the Escalating Manager is informed and the Webroster system is updated when an Emergency Response Visit is completed Updating notes to reflect all calls and contacts received and actions taken during out of hours Escalating any serious issues, incidents or safeguarding to the Senior Manager on call Supporting Community Compliance Supervisors with quality assurance Providing administrative support to the Care Manager or Community Compliance Ensuring there is a detailed handover given to the office team once the office is operational About You To be successful in your application as Waking Nights Support Worker, you will need to have / be: Access to your own transport – essential Level 2 NVQ would be advantageous Able to work as part of a team Able to write and understand risk assessments Experience of providing social care services and/or working in the social care sector Experience of writing Care Plans with outcomes and a person-centred approach Experience in carrying out reviews of clients Excellent time management, resource management and decision-making skills Excellent customer care skills with the ability to display empathy and warmth Excellent written and verbal communication skills Willingness to undertake personal development essential to the role Knowledge of the Health and Social Care Act 2008 and Care Act 2014 would be advantageous Hours & Salary As Waking Nights Support Worker, you will work an average of 38.5 hours per week from 8pm to 8am on a 3 on 3 off shift pattern There is a salary on offer of £20,000 to £22,000 per annum depending on experience and mileage is paid at £0.25 per mile.
