Due to continued success the company is seeking to recruit a highly motivated and enthusiastic Practice Nurse . The successful candidate will be responsible for the delivery of general practice nursing to the whole practice population. JOB ROLE (not limited to): Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being. Assess, plan, develop, implement and evaluate individual treatment plans for patients with a known long-term condition such as Hypertensive patients, Cardiovascular patients, Diabetic patients and Asthma/COPDK patents. Work with other health care professionals to prioritise, monitor, manager and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs. Review medication for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence-based practice and national and practice protocols. Provide information and advice on prescribed or over-the-counter medication on compliance, medication regimens, side-effects and interactions. Support and manager health needs of women presenting for family planning and cervical cytology consultations. Implement and participate in vaccination and immunisation programmes for both adults and children. Advise, support and administer vaccinations where appropriate for patients travelling abroad. Promote and deliver evidence-based care for patients presenting with aural conditions. Keep an up to date Smear fail safe that is available within the practice at all times. Communicate smear results to patients in an appropriate and timely manner. Adhere to all company policy in regards to Equality and Diversity, Health and Safety, Infection Control and Confidentiality. THE IDEAL CANDIDATE: NMC registered 1st level nurse qualification is essential. It would be desirable to hold a relevant nursing /health degree. Minimum 3 years post registration experience and least 1 year’s recent primary and community nursing experience would be desirable. Community nursing specialist qualification or equivalent would be an advantage. Experience of nurse-led management of long-term conditions. Experience of implementing protocols and clinical guidelines. Possess clinical leadership skills. Have excellent communication skills, both written and verbal. Possess negotiation and conflict management skills. Knowledge of health promotion strategies and local and national health policy. Be able to work in a team and on own initiative with the ability to work across boundaries. Live within a commutable distance and have the right to work in the UK.
