Recruiting for a Mental Health Night Support Worker to join our Young Peoples Service in Acton GBP 21,944 per annum, 40 hours per week plus 25 days Annual Leave, Pension schemes, Staff discounts, healthcare and more Minster House is a specialist semi-independent living (SIL) service for young people in and leaving care aged 16-21 providing a high level of support to young people with a variety of complex needs. The service supports six young people who can be referred from any local authority. The service is staffed 24/7 by a team of Support Workers and Night Support Workers. Night Support Workers enable people with particular needs to manage their tenancies and live independent lives of their choosing in the community. Night Support Workers are responsible for ensuring continuity of a support service at night and for keeping customers and the building safe. Key Responsibilities: - Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Contract Manager - Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions - Create support/action plans for providing appropriate services based on the assessment and maximising the use of resources available - Support customers to carry out domestic tasks, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation - Develop and maintain links with key agencies and service providers in the local community - Empower customers to ensure they receive the service and benefits they are entitled to - Undertake all administrative work and keep accurate and detailed tenant/customer records to high standards - Encourage and enable tenants to pay their rent and service charge ensuring that rent accounts are managed effectively - Ensure that referrals are sought and assessed proactively so that properties are filled as efficiently as possible to house those in need - Adhere to Look Ahead's Policies and Procedures - Engage in learning and development activity to increase knowledge and skills - Undertake any other duties consistent with the grade and nature of the post as may be reasonably required.
