Overview
Dementia Navigator - Permanent - £25,801 Southend-on-Sea Borough Council are currently looking for a permanent Locality Dementia Navigator to provide wraparound support and a single point of contact for advice and information in the community to anyone pre, peri and post diagnosis of dementia through to end of life, using national frameworks and strategies for ACP/EOL care planning to those who may be; frail, experiencing depression, anxiety, loneliness and social isolation. This includes carers, families and those known to the person. Work with the Team Manager and Commissioners to support the aims of the Southend 2050 Ambition, themes and outcomes and to support the dementia diagnostic pathways and the overall transformation of dementia services in Southend as part of the South East Essex Dementia Model. To work with Social services, Acute and Community Dementia services, Single Point of Access Team, Primary Care Networks, Care Homes, Domiciliary Care Agencies, Community groups and third sector agencies to enable people to achieve the outcomes which are important to them. What you'll need to succeed To be successful on this position, you must have the relevant qualification in Community Development / Education, Health or Social Care. Skills and experience in influencing, managing and supporting change at individual, family, community and systems levels. A good understanding of the needs of people with dementia and their carers. Experience of working alongside and supporting people and families from a variety of backgrounds and with varying abilities, strengths and needs. Experience of building partnerships with people, families and communities. Ability to apply a strengths-based, asset-based approach to assessment, care planning and review. Knowledge and understanding of the relevant strategies in place both nationally and locally for Dementia, Older Adult Mental Health, Social Isolation and Frailty.
