Job Description

District Nursing Team Leader - BAND 7 (FULL TIME ROLE) We are looking to recruit a highly motivated and enthusiastic Senior Nurse who has the ability to adapt to constant change. You will hold the District Nursing Specialist Practice Qualification or hold the Practice teacher:- District Nursing qualification. You will also need the skills to support service developments and move changes forward. This must be demonstrated in your application. This post offers an excellent opportunity to be part of our innovative District Nursing Teams. The District Nursing teams include District Nursing working alongside specialist Long Term Conditions Teams . The service is provided across Bexley. This post is to lead and manage a District nursing team You will work alongside the various professionals to implement a timely and person centered assessment to determine appropriate care pathway for patients to ensure the best outcomes and experience for patients. This involves partnership working across agencies at the interfaces of primary and secondary health and social care A key component of this role is liaising with GPs, ensuring that Multidisciplinary working and reviews are embedded within the service You will need the skills to support further service developments and assist in moving the changes forward, including strong and innovative leadership qualities. You will be working with the Locality Care Network (LCN) liaising with other physical, mental and social care colleagues The role combines line management for the nurses within the team and to ensuring mechanisms and processes are in place and maintained, you will also act as a source of specialist and expert nursing knowledge and provide support and advice to the wider team. You will be expected to champion the 6 C's and ensure these are embedded in the care of your patients. You will need to demonstrate excellent verbal and written communication skills. You will need to be flexible and enjoy working in a team. You need to have the ability to meet challenges and handle them in a professional manner.
