Job Description

We have exciting changes happening in the Community Diabetes Team and we are looking for exceptionally motivated Registered Nurses to join us. The Community Diabetes Service is an enthusiastic and forward thinking team delivering high quality specialised care to adults living with diabetes in Lincolnshire. We provide evidence based diabetes care which is able to meet the complex needs of individuals accompanied by the support of a skilled MDT that includes dietitians, GPs, Consultants Neighbourhood Teams and specialist nurses from other disciplines. If successfully appointed, you will be supported to develop your knowledge and skills in diabetes nursing as set out by Trend UK "An Integrated Career and Competency Framework for Diabetes Nursing" (TREND UK 2015) and advocated by Diabetes UK (Career and Education Framework for Diabetes Specialist Nursing, DUK 2019). As well as an interest in the care of people with diabetes, you will have a willingness to develop the clinical skills and competencies need to become a specialist and advocate in your field and to undertake post graduate education leading to PG certificate or diploma in diabetes. You will gain experience and exposure to a wide range of clinical skills in relation to diabetes care including recognising frailty, co-morbidities, dementia and management of complex health needs that may impact on diabetes management. The knowledge and clinical skills that you develop will equip you to progress into a skilled and competent clinical nurse specialist. This post is for 21hrs per week and will be based in the North East of the county (Horncastle / Louth) You will have excellent interpersonal and communication skills and be able to prioritise tasks. The ability to manage time effectively and work flexibly within the team are essential and you will need to be able to commute countywide at times to meet the needs of this role. You must be able to travel across the County to locations not served by public transport. Be Part of Our Team...
