Job Description

This is an exciting opportunity for a Band 6 Dual Diagnosis Practitioner to join a Liaison Mental Health Team as a Dual Diagnosis worker. It is excepted that the post holder will help to develop the pathway and build links with acute trust colleagues and substance misuse services and other agencies. Working with patients who present with complex mental health/substance misuse needs providing mental health assessments and managing risk/engaging with substance misuse services. The post holder will also aim to work closely with our frequent attenders worker/pathway and build relationships with other agencies to ensure patient safety within the borough and address complex needs/management with the multi disciplinary team . The Liaison Mental Health Team is based at Royal Oldham Hospital in newly refurbished offices and operate a 24/7 rota, however this role is expected to be between the hours of 9-5 across 7 days.
