With a CQC rating of GOOD across all 5 Key Lines of Enquiry, you would be working in a forward thinking hospital supporting our patients recovery alongside a highly skilled and dedicated Multi-Disciplinary Team. Taking a lead on our physical health strategy as the site's Physical Health Nurse Burgess Hill you will be working within a team that includes a range of professionals making an effective contribution to providing high quality care to our patients. You will be supported to work within an identified career development pathway and will be supported with peer review, clinical supervision, mentorship and appraisal. The post holder will be responsible for the provision of delivering and working in partnership with MDT's and the contracted Primary Care Service on the management of physical health care, in particular: health promotion, self harm management and chronic disease management including the development and implementation of care planning for our patients in regards to these needs. The post holder will provide a high quality source of advice and expertise in relation to self-harm, medical issues and the promotion and teaching of good health. This is a developed role and therefore has scope for innovation and development. Our Nurse Practitioner role involves: Multi health care service interface Expertise care planning Healthcare assessment, triage and signposting Interventions CQUIN Targets Teaching and nurse development To be successful in this role you will need to be committed to Personal Learning and Development, Evidence and Values Based Practice and Patient Centred Care. You will need to have excellent team working skills and be highly self-motivated with a flexible approach.
